Care home advisory service

We support care providers to manage medicines within their care homes and hospices, and we support primary care networks (PCNs), local authorities, ICS Place based medicines and quality teams to improve medicines assurance.

Our service can be tailored to meet your specific needs and include:

  • System reviews of all medicines related processes including safe and secure handling of medicines audits
  • Policy and protocol review – we have expertise within the team that can support assurance and quality improvement
  • Training webinars and education programmes to improve medication safety, for example training on documentation of Medication Administration Records (MARs) or on how to reduce medications errors and improve record keeping
  • Support with implementing proxy-ordering
  • Clinical Services and Structured medication reviews
  • Strategic pharmacy support and advice

All audits and recommendations are in line with current Care Quality Commission (CQC) and National Institute for Health and Care Excellence (NICE) guidance. Audit outputs include a detailed summary report, bespoke action plans, and solutions to drive improvements.

We have a care home Medicines Optimisation team who can support care homes and commissioners to deliver efficiency, safety, and quality improvement for the use of medicines to benefit staff and patients.

Our publications section, provides a range of case studies in relation to care homes and hospices including; Medicines Optimisation in Care Homes (MOCH) in the Black Country STP, medicines management and governance support for the Sue Ryder charitable organisation.
